第二型鈉-葡萄糖共同轉運蛋白抑制劑（sodium glucose transporter 2 inhibitors,
SGLT2i）除具有降血糖作用外，也有心血管與腎臟保護等多重效益。多個大型臨床試驗已證實其能顯著降低主要心血管不良事件、心衰竭住院及慢性腎病惡化等風險。
糖尿病與慢性腎臟病等醫學會均建議將SGLT2i納入治療計畫。2025年美國糖尿病學會（ADA）建議1，第二型糖尿病（T2DM）病人若合併或具高風險動脈粥狀硬化性心血管疾病（ASCVD）、心衰竭（HF）或慢性腎臟病（CKD），應給予SGLT2i以控制血糖並降低心血管與腎臟風險。2022年台灣糖尿病學會強烈建議T2DM病人使用SGLT2i，以預防糖尿病腎臟病及心衰竭惡化2。2024年全球腎臟病改善組織（KDIGO）亦推薦具CKD的T2DM病人及eGFR≥20且具明顯蛋白尿或合併心衰竭的CKD病人使用SGLT2i，以降低腎衰竭、心血管死亡及心衰竭風險3。
心臟疾病相關醫學會亦提出類似的藥物使用建議。2022年美國心臟協會（AHA）則明確指出，慢性有症狀的HFrEF病人，不論是否合併糖尿病，均應使用SGLT2i以降低心衰竭住院及心血管死亡風險4。2023年歐洲心臟病學會（ESC）指引亦推薦T2DM合併ASCVD、心衰竭（HFrEF、HFmrEF、HFpEF）或CKD（eGFR
≥20）者使用SGLT2i（如dapagliflozin、empagliflozin、canagliflozin等），有效減少心血管死亡、心衰竭住院及腎臟病進展風險5,

整合SGLT2i各種成分的樞紐試驗(pivotal trial)(EMPA-REG、DECLARE
TIMI-58、CREDENCE、VERTIS-CV、SCORED、SOLOIST-WHF)的安全性資料結果顯示相較於安慰劑組，SGLT2i可減少2%的任何副作用風險。此外，SGLT2i相較於安慰劑組不會顯著增加低血糖發生率，但會顯著增加8%泌尿道感染風險與減少22%急性腎衰竭風險7。
